Nominations

Stage One: Nomination submission is via the Community Services & Health Industry Training Awards & Conference nomination form. This needs to be completed for each nomination.

Stage Two: All applications need to be accompanied by evidence from the nominee. This evidence may be written, but can include copies of documents, video/audio tape, photographic display or some other means for conveying the evidence that demonstrates the nominee's capabilities against the selection criteria.

Please forward any hard copies or physical evidence to the CS&H ITB as soon as possible after submitting the online application.
